Notifying the Court of Address, Contact Information, or Name Changes
Notice of Change of Address and/or Other Contact Information
If you are involved in a court case, it is your responsibility to make sure that the court and all parties in the case have your current address and other contact information, including your email address and phone number.
You can use the form provided below and file it with the court. As with all other documents filed with the court, you must also serve all other parties in the case, unless you need to safeguard your address. URCP 5 governs service.
Notice of Change of Name
If you change your name while you are involved in a court case, it is your responsibility to notify the court and all parties in the case of your new name.
You can use the form provided below and file it with the court. Be sure to explain why your name has changed. For example, you may have gotten married, or you may have gotten divorced.
